I agree to return items borrowed in good condition in allotted time. I will be responsible for any repair or replacement costs. Loan periods are 45 days for DVDs, VCR tapes, microfilm, books and NAWCC BULLETINS.

Only two recent VCRs/DVDs (Nos. 800-) may be borrowed at a time.

I enclose correct postage in check, cash, or money order only: $4.00 for a single item, $5.00 for two, and $6.00 for three. NO CREDIT CARD CHARGES ACCEPTED. (Members outside the USA, contact the Library for rates and payment method).
